If you are uninsured or prefer to pay out of pocket, we welcome cash, checks and credit cards. Lab tests, additional services and visit complexity may result in additional charges. Pricing subject to change.and after hours price will be charge after 5:00 p.m.

need insurance forms, which are supplied by your employer or your insurance company.
