If you are uninsured or prefer to pay out of pocket, we welcome cash, checks and credit cards. Lab tests, additional services and visit complexity may result in additional charges. Pricing subject to change.and after hours price will be charge after 5:00 p.m.

If there is a question about your insurance coverage, a member of the Patient Financial Services department will contact you or a member of your family while you are here.

If you have health insurance, we will need a copy of your health insurance card and proof of residency. We also may

need insurance forms, which are supplied by your employer or your insurance company.
